Company Policies and Terms
  1. Payment Terms:

KAW requires a card on file to be provided before services are scheduled. In some cases prepayment may be required. Payment at the time of service can be made via cash, check, or charge. There is a 3.85% discount for all cash and paper check payments. Returned checks are subject to a $35 charge.

  1. Cancellation Policy:

Clients are required to cancel services at least 24 hours in advance to avoid charges. Failure to provide timely notice or failure to arrive for the scheduled service will result in a fee charged to the card on file, ranging from a minimum of $30 to the full cost of the service. We appreciate your understanding and cooperation in this matter. Timely cancellations allow us to accommodate other clients and optimize our scheduling. Thank you for your attention to this policy.

  1. HIPPA Confidentiality

KAW uses HIPPA policy to protect the Client's personal health information and treatment history. KAW will never share a Client's personal health information without written approval by the Client, the exception being when it is require by law. The Client has right to view their confidential file at request, and any charges for printed copies are responsible to the Client.

General Massage Terms
  1. Medical Disclaimer:

The Client acknowledges that massage therapy is for stress reduction and muscle tension relief and is not a substitute for medical care, diagnosis, or treatment. The Therapists at KAW cannot diagnose illness/disease, and any advice given shall be taken as professional opinion and not diagnosis.

  1. Client Health Disclosures:

Clients must inform the therapist of all known medical conditions, medications, allergies, and any other relevant health information before receiving services. A digital Health History form will be emailed to the Client prior to each session. Please use this to update any medical conditions. Clients arriving exhibiting symptoms of illness including but not limited to fever, chills, congestion, coughing, sneezing or headaches may be asked to reschedule and may still have to pay for services.

  1. Therapist Discretion:

The Therapist has the right to decline service or terminate a session if they believe it poses a health risk to the client or themselves, or if the client has not provided necessary medical information.

  1. Client Conduct & Expectations:

Clients are expected to behave appropriately, maintain good hygiene, and treat staff and other clients with respect. The Therapist reserves the right to terminate a session for reasons including but not limited to inappropriate touching, vulgar language, harassment, or intoxication.

  1. Consent:

The client gives informed consent for the massage therapy session understanding that, because massage therapy work involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19. The client also understands that they can terminate the session at anytime if they feel the service is a danger to their health or the therapist is using inappropriate touching, vulgar language, harassment, or is intoxicated.

  1. Scope of Practice:

The Therapist will only utilize massage techniques within the scope of their training and licensure. Each Therapist is licensed by the State of Michigan to provide massage services as defined by the State.


General Personal Training Terms
  1. Medical Disclaimer:

Exercise is inherently dangerous, and accidents can result in serious injury or death. The client is fully responsible for their own safety while performing any exercises and KAW is not liable for an individual's actions that result in injury or death.

  1. Client Health Disclosures:

Clients must inform the Trainer of all known medical conditions, medications, allergies, and any other relevant health information before receiving services. A digital Health History form will be emailed to the Client prior to each session. Please use this to update any medical conditions.

  1. Trainers Discretion:

The Trainer will only utilize training techniques within the scope of their education and certification.

Membership Terms of Service

The Member agrees to adhere to all membership conditions as are listed below or to be implemented in the future.

● The initial term requirement of memberships is 3-months. After this period the membership is consider month-to-month and each membership payment is considered a renewal of membership terms.

● Service “Credits” received with membership plans are non-refundable, non-transferrable, and expire at the end of the month of membership.

● Cancellation and “No-Show” policies apply to credited services and may result in loss of service credit.

● If a credited service must be canceled on behalf of KAW, the credit will be applied towards the rescheduled date.

● The Member has 3 days from the date of any membership agreement to cancel without penalty and a full refund of any charges to the Member in relation to this agreement will be issued.

● The Member can pause or cancel their membership by emailing contact@kzooathleticwellness.com. Please give at least 5 days notice of changes. All changes must be requested in writing.

● The Member may pause their membership for up to 3 months with no charge. At the end of the 3-month period payments will automatically resume.

● Payments will be automatically charged via credit card on file on the 1st of each month of membership.

● In the event of a denied payment the member must pay full price for all sessions, from the 1st of the month of denial on, until recurring payments are reestablished.

● KAW uses a third-party program to auto-charge accounts for membership dues and cannot be held liable for potential cyber-attacks against that company’s programming.

● A written copy of these terms must be signed by a Member before membership services can be redeemed.

Event Service Terms

Event service terms are provided to each potential account before scheduling any event service. These terms are to be signed by both parties before services can be provided. A copy of these terms can be requested by the account contact person at any time via email at contact@kzooathleticwellness.com
